Family Support Services

Family Support is a family-driven, team-based process for planning and providing support and strategies for success. Through the family support process, the team creates a plan designed to meet the unique and holistic needs of the family. Each family support team consists of the youth, the parents/caregivers, community partners, who meet regularly to implement and monitor the plan of action.

 

Emergency Resource Request Form

Family Support Coordinators work with Jewish Family Services and other community partners to secure items for FPS families in need. All items are stored in a Family Resource Room at Central Office and are provided to families free of charge.

  • FPS Admins, Support Staff, and Teachers may complete the form below to request resources for families in need of clothing, food, shoes, backpacks, etc.
  • Family Support Coordinators will fill the order based on what is available in the Resource Room and the items will be delivered to the school based on the request.
  • A maximum of five recipients/people can be entered in each form. This form is open for extended family members also.
  • If You have any unused clothes you would like to donate to the closet or have any questions please reach out to Damaris Madina-Hernadez the Quality Assurance Manager at Jewish Family Services at dmedina@jfsmw.org
