The following message is being sent to all FPS Families who are returning to in-person learning.
If you chose to return in person and do not know your assigned Cohort, please check your email or contact your child’s teacher or principal.

Before we get you started with the MyMedBot app, we are sharing the questions that you will be asked each day your child is scheduled to come to school. Please complete the questions every day they’re expected to in a school building, even if they’re going to be sick or absent. You will not be prompted to complete the MyMedBot Form Questions on your child’s remote weeks.
When you open the app and choose your child - the following questions will be displayed. Please answer the questions truthfully and thoughtfully. Your honest response and commitment to keeping your family members home when they are exhibiting symptoms will help to keep our community safe. Remember, if students cannot come into school, they can still access their classes remotely.
QUESTIONS TO BE ASKED
1.In the last 24 hours, has the student experienced any of the following symptoms:
a. Fever, chills or shaking chills
b. Temperature of 100.0 F (37.7 C) or higher
c. Loss of taste or smell
d. Cough
e. Shortness of breath or trouble breathing
f. Headache
g. Sore throat
h. Fatigue or unusual tiredness with one other symptom
i. Body pain or ache
j. Nausea, vomiting or diarrhea
k. Runny nose or congestion
l. None of the above
2. Has the student been tested for COVID-19, other than regular weekly in-school testing, and waiting for results?
3. Has the student tested positive for COVID-19 or has been diagnosed by a healthcare provider with COVID-19 within the past 10 days?
4. Has the student been told to quarantine or been in close contact with someone positive for COVID-19 in the last 14 days?
5. Has the student been in close contact with someone experiencing symptoms of COVID-19?
6. Has the student been outside Massachusetts to another state or country in the last 10 days and not been cleared by the school nurse?
COVID Call Center: Framingham Public Schools has launched a trilingual COVID Call Center to help with MyMedBot App questions. You can reach them by email COVID@framingham.k12.ma.us or by calling 508-782-6633.

- February 18th - February 19th: 6am - 3pm
- Beginning February 22nd, Monday through Friday: 6am to 6pm

Antes de comenzar con la aplicación MyMedBot, estamos compartiendo preguntas que se le harán cada día que su hijo(a) tenga programado venir a la escuela. Por favor complete estas preguntas cada día que se espera que ellos estén en el edificio de la escuela, incluso si ellos van a estar enfermos o ausentes. No se le pedirá que complete las preguntas del formulario de MyMedBot en las semanas remotas de su hijo(a).
