The MetroWest Community Health Needs Assessment (CHNA)

The following information has been shared with the Framingham Public School District with the hope of increasing responses within the MetroWest community. The Community Health Needs Assessment (CHNA) is a strategic plan for the entire healthcare system that is broken up by region. It's critical work that helps identify ways to create greater access to meeting the health needs in our community. 

Health care and community organizations in the MetroWest region are coming together to plan how health and wellness programs can best serve area residents, and they want you to be part of this important effort.

They are asking residents who live or work in Framingham to provide feedback and suggestions about health services and issues in the region by completing this 10-15 minute survey by March 31, 2019.

All responses are completely anonymous. There are no right or wrong answers; it’s your opinions that matter! Thank you for your time and contributions to this work!
