FORMAL REGISTRATION PROCESS CLOSED ON APRIL 10, 2015
1 - A copy of your child’s most recent physical (MUST BE within the last 12 months)
and Immunization Records
2 - Your child's birth certificate
(Long Form/Government Issued)
3 - Photo ID of Parent or Guardian
4 - A Utility bill dated within the past 30 days
(Not cell phone or car insurance)
5 - ONE OF THE FOLLOWING:
A current Mortgage Payment or
A current Lease or Section 8 Agreement or
A current Property Tax Bill or
A Deed or a Purchase & Sale Agreement
Residency Affidavit (If you are living with a relative or friend).You and the owner/renter
must complete a Residency Affidavit (click to print) and have it notarized.You also need
to submit two item as a proof of residency in Framingham (items 4 and 5 from the list above),
showing the name and address of the person with whom you are living.
Please call before your appointment if you need a Residency Affidavit and it will be sent to you.
A notary is available at the Parent Information Center.
*Homeless families are encouraged but not required to provide these documents.
