After processing your form, we will send you an e-mail (or letter if you
do not provide us with an e-mail address) with your registration
appointment date at the Parent Information Center.

Parents or guardians must provide the following items at the
registration appointment:

  1. Proof of residency
    (All applicants must submit two documents,
    one from each column below)
           
     Column A
                                         

     1.Mortgage payment
                  
     2.Property tax bill                         

     3.Purchase and Sale                                 
     4.Copy of deed                                         
     5.Lease                                                  
     6.Letter from Landlord                            
     7.Residency Affidavit (If you are living with a relative or friend)
        You and the owner/renter must complete a Residency Affidavit
        and have it notarized. You also need to submit two documents,
        one from each column, showing the name and address of the
        person with whom you are living.
        Please call before your appointment if you need a Residency
        Affidavit and it will be sent to you.


    Column B
    A utility bill dated within the past 30 days, such as:
    1.Gas Bill
    2.Oil
    Bill
    3.Electric Bill
    4.Home
    phone bill (not cell)
    5.Cable bill

     
  2. Your child's birth certificate (long form/government issued)

     
  3. Health Information -Immunization record &
    Completed Health History forms

  4. Photo ID of parent or guardian
    (A parent or guardian must sign the application.)

     
  5. Individual Education Plan (IEP) or 504 Plan
    (if child receives Special Education Services)

     
  6. Proof of Guardianship
    (if child is not living with one or both natural parents)

     

*Homeless families are encouraged but not required to provide these documents.



Testing: Students whose first language is not English will be tested
at the registration appointment to determine their English proficiency.
Program recommendation will be made according to the test results.
