* Year of graduation (If prior to 1992, was it North or South High School).
* Telephone number you may be reached at.
* If you would like an Official Transcript it would need to be mailed directly to college/university or place of employment. (Please supply address and contact information)
* If you would like an Unofficial Transcript, it may be mailed directly to you. (Please supply this address). Unofficial Transcripts are identical to Official except they don't have a school seal on them.
